This program is tailored to every patient's specific needs and situations.  Initiation into the weight loss program includes a thorough health and phsycial examination during your first visit.  We will disclose anything that may interfere with your weight loss journey.  A history of present illness and current medications will be discussed.  A review of labwork may be necessary such as a complete blood count, comprehensive metabolic panel, fasting lipid panel, thyroid panel, hemoglobin A1c and Insulin.  A copy of previous labwork within the last 12 months may be uploaded into the patient portal for the healthcare provider to review or the provider can order labs to be sent to your preferred lab if the healthcare provider decides that labwork is necessary after initial assessment.  The provider will discuss different options available for weight loss that the patient qualifies for and provide all necessary educational materials.  A body composition analysis will be completed monthly for the first 4 months and will be repeated as needed during the program.  This is used to monitor water weight, basal metabolic rate, and subcutaneous fat.

WEEK BY WEEK MEDICATION DOSE GUIDE

SEMAGLUTIDE:

  • Week 1 to 4: give 0.25 mg subcutaneously once weekly.  Starting at a low dose will help reduce GI symptoms that are associated with initial therapy.  After the initial dose is introduced, then you will start titrating up on the medication in the upcoming weeks.
  • Week 5 - 8: 0.5 mg subcutaneously once weekly
  • Week 9 - 12: 1 mg subcutaneously once weekly
  • Week 13 - 16: 1.7 mg subcutaneously once weekly.
  • Week 17 and onward: 2.4 mg subcutaneously once weekly

TIRZEPATIDE:

  • Week 1 to 4: 2.5 mg once weekly 
  • Week 5 - 8: 5 mg once weekly
  • Week 9 - 12: 7.5 mg once weekly 
  • Week 13 - 16: 10 mg once weekly
  • Week 17 and onward: 10 mg or more once weekly depending on patient progress and medication tolerance.

Dosing escalation will be delayed by 4 weeks if patient is not tolerating medication.

WHAT SIDE EFFECTS MAY I NOTICE WHEN I START THESE MEDICATIONS

Side effects that should be reported to the provider ASAP:

  • Dehydration - incresed thirst, dry mouth, feeling faint or lightheaded, headache, dark yellow or brown urine.
  • Allergic reactions - itching, hives, skin rash, swelling of the lips, face, tongue, or throat.
  • Change in vision.
  • Hypoglycemia - shakiness, sweating, blurred vision, seiures, coma, dizziness, confusion, irritability, heart palpitations, fatigue, weakness.
  • Heart palpitations - rapid, pounding, or irregular heartbeat.
  • Gallbladder problems - severe stomach pain, nausea, vomiting, fever.
  • Kidney injury - decrease in the amount of urine, swelling of the ankles, hands, or feet.
  • Pancreatitis - sudden and intense upper abdominal pain that may radiate to your back, nausea, vomiting, fever, rapid pulse.
  • Thoughts of suicide or self-harm, worsening mood, feelings of depression.

Side effects that do not usually require medical attention (report to the healthcare provider if they are bothersome):

  • Loss of appetite
  • Diarrhea
  • Nausea
  • Stomach pain
  • Vomiting
  • Hair loss

**Initial visit includes precribed daily supplements/vitamins to help prevent hair loss and other possible bothersome side effects.  Will provide Zofran as needed for nausea/vomiting.

 

WHERE SHOULD I STORE MY MEDICATION:

Store medication in the refrigerator.  Do not freeze the medication.  The medication should be kept in the original container until you are ready for your scheduled dose.  Make sure this medication is kept out of reach from children and pets.

CONDITIONS THAT NEED TO BE DISCLOSED BEFORE STARTING THIS MEDICATION:

  • Eye disease or any vision problems
  • Endocrine tumors (MEN2) or a family history of these tumors 
  • Mental health status, depression, suicidal thoughts, plans or previous attempts
  • Gallbladder disease
  • Kidney disease
  • History of pancreatitis
  • Stomach or intestinal problems
  • Thyroid cancer or family history of thyroid cancer
  • Unusual allergic reactions to GLP-1 medications, foods, dyes, or other preservatives
  • Pregnant or trying to become pregnant
  • Breast-feeding
  • Insulin or other medications for diabetes

The provider will request a list of all medications, herbs, non-prescription drugs, or dietary supplements you use.  The provider will inquire about smoking, alcohol intake, or use of illegal drugs.  This is a safety precaution to make sure drug interctions do not occur.

I MISSED. DOSE, WHAT SHOULD I DO NOW?

     If a dose has been missed and the next scheduled dose is MORE than 3 days away, the missed dose may be taken as soon as possible.  If you missed a dose and the next scheduled dose is LESS than 3 days away, do not take the missed dose.  The next dose should be taken at the regular scheduled time.  Extra or "double" doses should not be taken.  If two or more doses are missed (2 weeks or more), take the next dose at your regular time or call/message the care team to discuss your options.

DIET/EXERCISE 

Diet and nutrition will be discussed at the initial visit and as needed through follow-up visits.

  • A meal plan will be created initilly and can be modified throughout the program as needed.
  • Your will have unlimited messaging and support from the care team, unlimited access to the patient portal where you can pload weekly accountability check-in sheets to be revieed by the provider and discussed at follow-up visits.
  • Personalized nutrition and lifestyle advice.
  • Access to weekly meal plans.
  • Ongoing nutrition education.
  • Content including recipes, podcasts, videos, etc.
  • Exerccise is recommended 2-3 days a week during this program.  A personal trainer is extremely beneficial for any weight loss program.
